UPCAP Care Management is a program for Upper Peninsula of Michigan residents who are candidates for nursing home care or are currently in a nursing home. The program helps people understand their choices when thinking about nursing home care. Those who choose to remain at home can receive help in obtaining needed services. Teams of Registered Nurses and Social Workers assess client's needs and, whenever possible, develop a care plan that may allow them to remain at home. UPCAP Care Management will then arrange and monitor the participant's services and make changes as needed.
UPCAP Care Management Provides: Long-Term Care Options, Comprehensive Assessment, Professional Recommendations to meet long-term care needs, Care Planning, Service Arranging/Coordination, Long-Term Monitoring and Re-Evaluation, and Client Choice Advocacy.
